How to Participate

When a pet dies or is euthanized at your clinic, the Veterinarians CARE Program offers a meaningful way to show your compassion. Participating in the Veterinarians CARE Program is possible by following these three simple steps.


We ask that clinics and practices wishing to participate in the Veterinarians CARE Program complete our registration form. Registration is a one-time process for each clinic or practice. If you have already registered, you can login and begin.

2Complete the Client and Pet Information Form

Once your clinic or practice has registered, complete the online form detailing each client and pet to be remembered.

3Make a Memorial Donation

After you have submitted a client and pet information form, you will receive a confirmation e-mail asking you to make a donation.

You may make your donation:

Online. Visit the Cummings’ secure online giving page. Select the area you wish to support from the menu or select “Other” to type in your preference.

Mail. Print and complete the Veterinarians CARE Gift Mail-In Form and send, along with your credit card information or check made out to “Trustees of Tufts,” to the address provided on the form.

The Veterinarians CARE Program kindly requests a minimum donation of $10 per memorial.

Upon receipt of your donation, Cummings School of Veterinary Medicine will mail the personalized letter(s) to your client(s), acknowledging that we have received a charitable gift from you given in memory of their pets. The program does not solicit your clients or enter names into our database, unless they independently give to Cummings or seek hospital services from the school for their pets.
